Medical Exemption
To request a medical exemption, you鈥檒l need a letter from your doctor, physician assistant, or nurse practitioner. The letter should explain why you can鈥檛 get the vaccine and state whether the exemption is permanent or temporary. If it鈥檚 temporary, it should include when you might be able to get the vaccine. The letter must be signed, dated, and written on your provider鈥檚 official letterhead.
Religious Exemption
To request a religious exemption, you鈥檒l need to submit a letter explaining how the vaccine goes against your sincerely held religious beliefs or practices. Note: If you are 17 or younger, a parent or legal guardian must provide this letter for you. This letter will expire when you turn 18, and at that time, you鈥檒l need to submit your own letter.
How to Submit Your Exemption Request
Please upload your exemption letter at Select UPLOADS from the side bar menu and then SELECT FILE next to MEDICAL EXEMPTION or RELIGIOUS EXEMPTION. You will be notified via your 51重口猎奇 email of the status of approval.
